Health Insurance and Concordia I.D. Card
Don't leave home without them! Please bring your health card and Concordia I.D. with you on each visit to Health Services.
In order to see a physician you will need to present proof of a valid provincial health card or International Student Insurance card. If you do not have a valid health card we will be happy to provide you with a physician appointment, but we will have to charge a service fee.
If you are unsure about your coverage, please come to Health Services at 1550 De Maisonneuve W. GM-200. We will provide you with an information pamphlet on how to obtain a valid health card.
