Don't leave home without them! Please bring your health card and Concordia I.D. with you on each visit to Health Services.
In order to see a physician you will need to present proof of a valid provincial health card or International Student Insurance card. If you do not have a valid health card we will be happy to provide you with a physician appointment, but we will have to charge a service fee.
If you are unsure about your coverage, please come to Health Services at 1550 De Maisonneuve W. GM-200. We will provide you with an information pamphlet on how to obtain a valid health card.
If you are an international student and would like more information on
insurance contact the International Student Office at 848-2424, ext. 3515.
To obtain information on your provincial requirements to maintain your insurance coverage, please refer to your province.
