If medical problems or concerns are interfering with your ability to attend class, complete assignments, or write exams, Health Services staff can work with you to assess the problem and review ways in which Health Services can be of assistance.
The following criteria must be met before Health Services can provide a medical excuse certificate:
- You must have been followed at Health Services during the time of illness. This means that you were seen at the Health Services prior to the exam, class or due date for the assignment. You must have been seen with sufficient time prior to the exam, class or assignment to ensure that any treatment plan prescribed by the health care professional can be initiated to optimize your being able to fulfill your academic responsibilities.
- If you are not followed by Health Services, we will be unable to provide documentation. Documentation must come from the external health care provider.
- That the nature of the illness is serious enough to justify absence from an examination, work, class, or warrants extension for assignments. The medical assessment must reveal objective signs of an illness.
- Forms that must be completed by a physician may incur an administrative fee.
